What kind of wallplate is this??

I need help identifying a single-gang white plastic wallplate that has two horizontal side-by-side insert holes and each vertical edge (side) of the plate has about a 1/2 inch wide convex curve to it. The jacks (inserts) looked like ortronics but I didn't get a chance to take it apart to look. I need to install some additional voice/data outlets in the same office so have to match.

Thanks in advance for any help. I need to find out what parts to get before I go back to do the job.
 
Many of the faceplate makers will mold a brand and P/N on the inside. I dug through a bunch of old faceplates looking for what you discribed. I have some old Panduit plates that curve out, but I don't think quite like you explained. ??????????????/

Take it apart and get the numbers off the parts and maybe we can be of more help.
 
OK...guess I'll have to go back and open it up and see what's there like I should have done in the first place.
