The most common reasons I have seen (in this order):

[li]PC shut down ;-)[/li]
[li]Windows XP SP2 firewall or other personal firewall[/li]
[li]Client received another IP address from DHCP server (in this case, the same client name will be in the list with another IP address as well[/li]
[li]SAV service not running on client[/li]
[li]Network issue between client and server[/li]
[li]Manual refresh needed in Symantec System Center (does not happen automatically)[/li]
[/ul]
