What causes SD card failure that requires it t be recreated? 1

I have a question that I could not find in the docs.. I have been wondering why or under what circumstances will cause an SD card to fail and have to be recreated. I see a lot of issues that are resolved once the card is recreated. What can prevent failures i.e. unplugging power cord, over heating cabinet, etc... Thanks

In my opinion mostly a corrupt file on the card... Why this happens... Perhaps a sub optimal I/O handling by the IPO.
