I am trying to create a web-based survey. I need drop-down boxes for users to select something as well as radio buttons for one to be selected and check boxes that will allow multiple selections. How can I create this so that the submit and reset button send all the results in one shot.

Please advise.
 
Hi sohtnax,

You can start with either Insert>>Form>>Form or Insert>>Form>>One line text box. Either of these will bring up the form surround(the dashed or dotted line & Submit & Reset buttons). Make sure all your fields are contained inside the lines.

Once you have everything inside, right click to bring up form properties, click Send to: at the top. This is where you set the email and file destinsations.

My preference is to set up the form and publish. Make sure it works, and then go back to set up validation.

Be advised, that the FrontPage form requires both the FPSE and publishing with FrontPage to http location.
