Web Form that creates a PDF

I am looking for a way to create a web form where a user can input information into the form, submit the data and a PDF is created with the data in barcode format where they can then print out this PDF.

Any ideas?

There are PDF writing APIs for nearly all server side languages. Choose one that is applicable to your environment and go to town with it.
