wanting a formmail script, no idea how to set it up

i have the script, im running an apache server on windows xp with perl capabilities, when i try to execute the script i get a 500 error and the error log says bad compilation.
.pl files work fine that came bundles with the server install, not sure where sendmail is located on the server or if it even has 1, im assuming it does,
can anyone help?

Bad compilation is a new one to me, given that its interpreted.

Can you be more specific w/ the error message
