Want an additional form page at end of each group in a report.

I'm a new SSRS user - using Visual Studio 2008.

I need to build a report that provides a list of contacts for an organization, like so:

Mary Jones
Director
ABC Company
123 Main Street
Anytown, CA 11111
(444) 555-1212
Mjones@abccompany.com

Scott Smith
ABC Company
Sales Associate
123 Main Street
Anytown, CA 11111
(444) 555-1212
Ssmith@abccompany.com

I'm grouping the report to break on each company, and need an extra form at the end of each group where the recipient can fill out text boxes to add additional contacts as needed:
