W3svc causes access violation

IE
on boot an access violation error in "0x63801e9e" error appears. I have reapplied SP6a to an NT 4.0 system and applied patches to IIS up to Q321599i.exe. the proxy is working correctly but the default web site will not start.

i have read numerous KB articles but none give solutions as such

any help much appreciated
