Voicemail Ring-back to Operator

I've got one of /those/ problems today with a customer. It's the usual, "It happens somedays and not others" type problem. It's an SX200-ICP, w/ embedded Voicemail. All I know is that the Operator is getting calls from "VM 1, VM 2, etc." on the display and when she picks it up, it immediately hangs up. I have been unable to reproduce this problem. Can anyone tell me a good place to start looking?
Site has Auto-Attendant on the main line, option 0 -> transf to ext. 5546. Option 1 -> Info, Option 2 -> Info. That's about it.

Make sure the VM Ports COS does not have option 604 enabled.

Also make sure the Line Preference (Via Form 9 Expand Set) is set to Manual.
