Voice Forms

I am looking for a system that allows people to complete a form by using voice over the telephone.

The idea is that they complete an application form by giving spoken answer to questions. These answers are then voice recognised and entered into a database.

Any body know of such a thing and if so can it work with Meridain M1 ?
