Couple of things:
1. Did you reset system after programming slot?
2. Do you have talk battery on phone?
3. Consider configuring station port for ringdown operation, or off-hook to analog trunk, just to confirm connectivity.
4. If you have an MPB1, you'll need DTRU4 DTMF receiver daughterboard
