VMPro 9.11 dial by name drops call after selecting the person 1

I have vmpro 9.11 with ipo 9.11
SIP trunks

Ive had several issues so far with earlier 9.xx releases.
Now, dial by name is dropping calls only when selected certain users.
I hope I'm doing something wrong.

example...
dial # for dial by name
I press #
I spell 645#. "For niki, press 1"
I press 1, then the call drops.

all users work fine but this and one more.
Ive changed the name for different mailbox and still hangs up.
Ive chaged the extension and still hangs up

I haven't changed the physical phone yet.
but it was working just fine before.

The user stated their name in the mailbox.
the system actually selects the correct person but drops the call when it about to transfer.

Any ideas with this one?

The Dial by name is just default on vmpro. I didn't add anything special to it.
