VLAN Tag resetting

Maybe I am overlooking something simple, but I installed an IP500v2 last week along with 2 Avaya switches that are in a stack. Phones were working fine last week up until this past weekend when they plugged in their data equipment.

Over the weekend, the phones started rebooting themselves. When I came in today, I noticed that the phones would boot up, grab the data VLAN (1), then boot and grab DHCP off voice VLAN (5), download the files from the file server, say contacting call server and then reboot. Occasionally it would let you login, but it would reboot within 30 seconds. When I go into the craft menu, I notice that the 802.1Q setting keeps turning to "off". When I turn it to yes or auto, and reboot, the phone will log back in for about 15 seconds, and then reboot, 802.1Q tag again off.

I have tried setting this in the 46xxsettings file to 0, which should be auto, but doesn't make a difference.

I unplugged their network from the Avaya switches, and performed this same test with the same result.

Any idea what could cause this?
 
Interesting... I'm having a similar issue with AVAYA switches in my stack.
So two VLAN's? DATA 1 and Voice 5

Yeah, 2 VLANs.. 1=Data, 5=Voice.

Option 242 in the DHCP Server is correct like you have.

I also tried this. I unplugged their data from the Avaya switch, I manually put the IP Phone into VLAN 5 without changing anything else(kept it DHCP), and the same thing happens. So it only happens when using DHCP from the IPO. Statically set the IPs and the phone works fine.
 
Here is what I use and it worked like a champ. Avaya switches and used the DHCP server on the IP Office for my vlan 3 (Phones only VLAN)
Windows DHCP Server entered on my routed VLAN other than IPO VLAN.
Option 176 MCIPADD=xxx.xxx.xxx.xxx,MCPORT=1719,TFTPSRVR=xxx.xxx.xxx.xxx,L2Q=1,L2QVLAN=3,VLANTEST=0
Option 242 MCIPADD=xxx.xxx.xxx.xxx,MCPORT=1719,HTTPSRVR=xxx.xxx.xxx.xxx,L2Q=1,L2QVLAN=3,VLANTEST=0

xxx.xxx.xxx.xxx is the IP Office LAN1 interface. I am using LAN2 for direct access to remote/home users via Internet.

Make sure you have IP Helpers for the non voice VLAN that point to the Windows DHCP server. Voice VLAN does not need the helper.

TFTP in IPO and HTTP use memory card.

Setting L2Q=1 turns on 802.1Q and L2Q=0 is Auto and L2Q=2 is off If I am using it turning it on is better.

VLANTest=0 is default value, but again like auto assures it is 0
Vlantest is setting the number of seconds to wait for a DHCPOFFER when using a non--zero VLAN ID

Yeah, nothing new there that I haven't checked already. I've configured these switches a dozen times for different customers. This one is just acting funny. Can't pinpoint it yet.
 
have you check switch code consistency.

Verify the code on your switch is the same as the ones you have had working in the past.

Are you using the new 4.4.1.0 code?

Also, did you plug in the data, if not verify they did not plug a data VLAN1 into a VLAN5 port. You can see this in the arp tables on the switch.
