Visual debug screen when opening word doc in IE

If a client opens a word doc in Intenet explorer - a visual basic debug screen pops up. This has happened before - and whilst attempting to fix prob - we went to SP6 for IE. This didnt work - so we renamed clients profile, and when she logged on again, new profile was created and problem was fixed. This problem has returned like the proverbial.... any ideas gurus?
