You order it from Nortel, or if you have a vendor they can order it for you.
If you order the "Upgrade Kit" a keycode authorization is included.
The authorization is used, in conjunction with your BCM system ID, to generate the keycode using the Nortel Keycode Retrieval System.
That's what I've been debating too. Ours is 2.0 (and I have a 1100 expansion module I can't use because of it). I wonder if patching it fully before upgrading to 3.0 minimizes the risk of failure.
We've done many 1.0 and 2.0 to 3.0 upgrades.
The only time we've had issues is when the upgrade guide was not followed by the person doing the upgrade.
