V9.1D08 ocx event occasionally fails to fire the Progress trigger

I have an application with an embedded ocx which routes telephone calls to the application (via a dialler) supplying an account number to my application. This will run fine for 10 - 20 times and then occasionally fail to fire the progress trigger code in the application.

Is there any known problem in this area? I found something similar in the Progress Knowledgebase for COM with DLLs and this suggested a move to 9.1E or 10.

Any ideas anyone?
