USP Disk replacement

We have a situation that has us puzzled. Every time we replace a failed disk, the system detects some kind of error and the new disk comes up as blocked. We can, however, restore the disk, and then restore the data from the spare without any problem. We have had this unit for a long time, and this is something that has just started recently. There is no other hardware failed on this unit.
