Using Outlook Forms

I am setting up a new form in Outlook 2003 and when I go to send it, the e-mail shows up in the inbox blank. When you open the e-mail item, it shows the form. I know there is a way to show in the preview window that it can't be displayed in preview, but must be opened. I just can't figure out how.
