Using modem for Voice to divert calls

Is it possible for me to use a modem to intercept voice calls on the phone line and then divert the calls to another number. Then be able to at a specified time divert the calls to a different number automatically? What sort of hardware \ software setup would be required?

Sounds like it should be easy, any idea's??
