Using Lotus Notes to do a feedback survey? 1

Hi all,

I'm trying to use Lotus Notes to do a user survey. I would like the form to be sent via e-mail and have users check boxes and send it back to me. So far by following tutorials I figured out how to create a html feedback form, but I've failed to figure out how to send the form and allow users to send it back. I would prefer that the e-mail be the actual survey form rather than having another clickable link. Is there any documentation on the web or has anyone had to do a similar project?

Check out the download section on Notes.net - there is a sample survey database on there which I use all the time. I have modified it slightly, but you can enter as many questions you want, the type of questions and where the responses should go. All answers are then categorised within the database. Even if you don't use it for your own project it will gove you an idea.
