Using Form and VBA for Username and Password

Can someone point me to the thread (or, provide code) regarding the code and instructions which uses an access form containing username and password fields, which is in turn cross-referenced against a table containing usernames and passwords to allow access to the DB??

I had previously used this code successfully, but have since lost it!

I believe it was originally posted sometime in mid to late 05.

You can use keywords to do an Advanced Search in the Access fora*.
