I have a form setup in a web site that works...but, due to my server being unable to install FP 2002 extensions, I had to create the form outside of FP...you're welcome to use it. it's url is:
Click the "Stateside Mission"...then the "Apply Here".

I made this form from this site: Good Luck!

Can anyone help me "help' my server succeed in install FP's 2002 extensions without paying Microsoft $5000???

Change your server to apex2000.net in Midland, TX.
That's who I use, and they have just changed to the latest Microsoft technology, including the latest FP extensions.
And they're good to work with, also..
And No.. I do not work for them.. Just a happy customer.
