Using Custom Forms on Win/NT 4.0 & Citrix Metaframe

We have an 11 1/2 x 8 1/2 form that was designed by non IT personnel.

How or what driver do I need on my Citrix server so my Win/2000 workstations can setup a custom form for either an Okidata 395 or an Epson DFX 5000 or DFX 8000 that is
11 1/2" wide and 8 1/2" high.

Need help badly on this one.

Do you need to input data, then print this custom form? Or do you just need to set size parameters? What language is it written in? MetaFrame 1.8, or an earlier version? I'm pretty good at printing issues in Citrix, but I just need more info.
