Using an Array to populate a Form?

Greetings. I am looking at populating a document form from a dialog in a very specific format. The form itself is court minutes and I've represented each line with a form field on the document, and each line on the dialog is either a combo box or a text box.

Can I use an array to specify which text value goes where on the form? It occurs to me that since each element in the array has a specific and therefore referenced location, I should be able to assign the locations per event and wipe them for the next one...am I just kidding myself?

My grid is 6 lines down and 3 columns across for a total of 18 grids per event. Either three seperate arrays or one multidimension?

Or is there a better way to populate text into form fields in a table?
