Using Adobe Acrobat to produce an survey/questionnaire attachment

I am totally new to Adobe, so i am just making some general enquiries as to its caperbilities. I need to conduct an email survey by sending out an email to various people, of whom which i do not know what email software they are using, containing a exe or peice of software that they can open and run through the questions put forward - send back and they are easily outputted to excel or acces or word or something. Does anyone have any ideas as to whether Adobe can do this, if so, where would be the best place to start? Any advice would be gratefully recieved Thank you for your help already.
