Use form value in email

I'm looking for a script that would retrieve someone's name entered in a web form and use it in an email to make it more personal.

Example, someone makes an inquiry using a web form:
Instead of a generic "Thank You for .... ".
I'd like to say "Dear Paul, Thank You for .....".


Thanks in advance for your help!
 
you can dunamicaly give the subject box the value of the name entered. This will appear then as the subject
example:

document.frm.subject.value = &quot;Dear &quot;+document.frm.Fname.value+&quot; ,....etc..
form.submit();
