Urgent Please

I have Designed a Form and I want it to Submit results to the Email not in the Database.
Unfortunately I don't know CGI/Perl or any code that will work properly. Can someone help with the code.

The Form I have designed it looks like this 1. I used JavaScript before to submit
the information but now is no longer sending Emails I don't know way.

FirstName
LastName
DOB
Cell
Email
Course
Duration

I want it to send information this way,

Firstname: James
LastName: Pieterse
DOB: 01/01/1982
Cell: 071888888
Course: IT
Duration: 2 years.

Can someone help with any codes, JavaScript, CGI and any code that will work properly please.
My deadline is Wednesday. Please

From what I know to send an email you have to use server side programming like, ColdFusion or sth similar to that.

what other backends can u use. i know PHP and ASP...
