Urgent Help With Create and Publish Forms

Hello Everyone,

I am having a project which requires form creation for the Exchange 5.5. I created the forms through Outlook 2000. And this is my very first project that deals with form creation in Outlook.

I have created a request form that will be filled out by a requestor and sent to the management department. And the management department needs to fill out ANOTHER form and send it to accounting department.

The problem I am having now is I cannot put the form for the management department to fill out on a public folder.

In the request form, I edit the Read Page and have an action button so that it grabs another form for the management department. In the (Actions) tab in the design form view. I clicked on the "New" button, a Form Action Properties window poped up. At the "This action creates a form of the following type" section in the window, there is a field, Form name, where I need to specify which form I want to use. However, I am only able to choose from "Standard Forms Library" and "Personals Forms Library". However, I am not able to publish a form in the "Standard Forms Library" and I need the form to be available to management department in a public folder.

I have created the request form in a public folder and others can use the forms in there. However, I am not able to use those forms from the "Form Actions".

I am hoping that I can explain the whole thing as clear as possible.
