Upload Form Redo Assistance

I've Completely re written the upload form, just can't get it to work at all can anyone tell me what im doing wrong, what its doing is storing the path of the uploaded file and Putting the File in the selected Folder from the drop-down. I've included the whole thing in a zip file, Including SQL
