Upload an image to an webserver and save the path to database

How do i upload a picture and give the picture an uniqe name and upload the picture into an WEBSERVER FOLDER and copy the path of that picture and put it into a database ...

I wanted to upload an picture of an person and if the user puts in his username and password he see the personal data and his picture ...
