upload acrobat form results direct to server

Someone told me that I can upload form results from a form I created on an Acrobat document directly to the webserver.

I googled but not really sure what I am searching for. The pdf is a survey we post out but it would be good to email out elecrtonic copy, have them save on their machine, fill out and press a button to upload results dirrctly to the server here.

Is this possible?
