How do I upgrade my Solaris HW version ?

It's now -

Solaris 8 HW 7/03

I want to upgrade to HW 2/04

Please ignore the version numbers, but is it possible to change the HW version on Solaris by patching it ?

I think the only difference is the patches. You should be able to get it up the latest release by patching it.
