Upgrading code on an RSM

I've got the generic procedure, and it worked on one node but the second one I tried I didn't get the prompt to erase the existing code and the download failed (Bad CRC check, file will not open try reformatting to squeezing).

So I format bootflash:
it formatted and came back fine

copy tftp flash
it uploaded with no errors this time

sho flash
CRAP!!!

Now I have:

RSM-A#show flash

How do I get rid of the other 2 entries? It's obvious the last is the one I want to keep, I tried:

erase flash (command not supported)
squeeze flash (didn't work)
