Upgraded to 4.1 on IP 406 now reboots randomly 1

I upgraded a cusotmers IP 406 from 4.0(10) to the new 4.1(9). This has been rebooting randomly since. I have looked on the Audit trail and it states COLD START which I know means its a power reboot. I have made sure that no groups overflow to the same group and made sure it is not an actual power issue. Any ideas
 
I have also notciced this error in System Status on the last restart is states it was an abnormal termination, the cause is TLB address=ff27fb98 d=1 pc=00000000 stack=fec64924. I am trying to capture with system monitor and it has not happened yet.
 
TLB = Bad

get a trace of a reboot and send to whoever supports you

 
Oh, i thought TLB = Total Loss Brother.
Nevertheless, it can only be decoded by Avaya, so follow TomMills advise : report to Avaya via a distributor.
 
what type of sets are you using?

i had an issue with a 406v2, 30 port digital module, and 5410 sets. i tried to put a 4424D+ set on the system to get more buttons with lights, and the system started rebooting immediately after i hooked it up. we took it off, and the reboot stopped.
 
Thanks for the advice.

The sets I am using are mainly 5410's I have 3 DS 30 modules and the other weird thing that is happening is that every now and then the VMProDB Service has to be restarted. This has hapeened since the upgrade but has only started to happen a week after.
