Upgrade to 11.1 Server Edition issue

Hi All,
I have a system that has primary and secondary servers which are at version 11.0.45
I have created the 256Gb USB key as per the documentation for upgrade to 11.1
I have upgraded the secondary server successfully.
When I try to upgrade the primary server using the same USB key it appears to be trying a new install instead of upgrade. I get the screens to select keyboard, language, accept conditions etc that were not presented when upgrading the secondary server. It is at this stage that I cancel and let the server boot back to 11.0.45
I have re-created the USB key three times now and have ensured that the two files are copied from USB folder to root of USB key and nothing else.
I am confused because it worked with the secondary server.
Any pointers from the experts on the forum would be most appreciated.

Are you upgrading to 11.1.2.2?

I have exactly the same issue with my IP Office SE 11.0.4.7

I tried the following version:
11.1.2.1
11.1.2.2
11.1.2.3

also tried different USB devices ( a stick, ssd drive). Always the same results.

MJCarroll: Did you manage to figure out how to solve this without clean installation ?

NOTE:
This IPO SE is based on older Dell R320XL. However this should not be an issue.
I got an another site with the same server model, that just worked fine. I am wondering what could cause this.
