Unsubscribe form

I'm not well versed in web forms but I'm looking to find an easy way to create a unsubscribe form that's web based. Basically people could unsubscribe to a mailing list and this gets dumped to a text file or DB.

Any help on an easy way to accomplish?
 
you could just set up a database to accept emails, or if you have one already (which i assume you have) have the user enter their email and either

1) it adds that email to your new database that you can export to a text file and cross reference or

2) they enter their email and it erases their email from your preexisting database.

If it were me I would try and work it out with option #2

Actually the emails get manually pulled from a contact management DB. Perhaps I show setup a Access DB and tie to a web form? IS there software that makes the process easy?
