Unixware 7.1 missing .index file with new users

I am cleaning up the users mail box area and found that 24 out of 142 of the users defined do not have a related .index associated to their mailbox. Does anyone know why is this index NOT created during the add of the new user to the system? I have found TA 112602 that explains how to fix the problem after the user is created, but I am trying to find out why it is not being created when I add the new user. Is there something I am not doing properly when adding the new user? Any help would be appreciated.
