When a UNIX system loses an active path what are some questions she/he ask the SAN team?

I was thinking something like this:

Is this fibercard (WWN #), online?
Can you do a 'light test' from HBA to Switch?
Is there any activity from this fibercard (WWN)?
If fibercard is down, when did it go down?

Should that suffice for a UNIX SA? or are there any other questions they need to ask.

I would agree with those.

The Unix SA may ask:

Did the masking/zoning change on the switch or storage?
Did the switch port go down?
Did the fiber cable fail?
