Set up key press #5 in atty's mailbox to transfer to alternate ext. (secretary's ext.) When the caller presses 5 the call is tranfered to the secretary's ext fine but she has no idea which mailbox it came from. She just sees the caller's CID.

Is there a way for her to see who's mailbox/ext. the caller presses #5 from to get to her so she can answer the phone: "Mr. Johnson's line"?

If this option 5 is just set up in one users voicemail you could have option 5 first goto a bogus CTI route point with the description setup as "Mr. Johnson's Line" and then setup that CTI route point to forward the call to the secretary's extension. This way the call will say "Forward from Mr. Johnson...." There may be another way to forward through unity but this idea just came to mind... Unity -> CCM Ext -> CCM Ext set to forward to Secretary X.

I just tried that but it still doesn't carry the Alerting name from the CTI RP.
