Unable to submit form

Whenever a user completes a form created with frontpage 2003 and clicks the submit button. The form does not take the user to a confirmation page, the form inputs are not submitted in text file in the _private folder rather the form is displayed again.

Pls how can i resolve this so that the form submits successfully. Am using FP server extension 3.0 and IIS 6.0
