Unable to configure SIP -Vitelity on BCM50 rel 6 with IP trunk keycode

I am trying to configure SIP carrier vitelity on a BCM50 release 6. I have the IP trunk keycodes. Vitelity uses name/password authentication. DTMF mode is rfc2833. I have the register string. I also have the SIP Proxy, Inbound server, outbound serverand domain name. Does anyone have a template for this?
