Unable to authorize payment: Missing Required Shipping Information

I cant figure this out. I cant get any payment info to show up. Credit card COD nothing. It says payment info then absolutely nothing show up underneath. I went into pages turned items on and off nothing.

I made sure shipping payment and order were configured. Still nothing.

Please help.
