Two incidents of name, address on one report?

I have one database that I want to get the name and address out of and on a report. It's a statement that the bottom tears off for them to send in with payment. I put the name and address fields on the top portion and then on the bottom portion but it comes up with different names each place and they should be the same. Not sure how to fix this? Please help!!

Try adding a second detail section with the name and address fields in it for the bottom section. I.E. something along the following:

Detail a {name}
{address}

------------------------------------------------------
Detail b {name}
{address}
Please return this portion with your payment.
 
If you group on the Account Number, you could then place the Customer Name and Address in the group header and in the group footer to create the tear-off section. If you go to Format Section->Group Footer and check "New Page After", you will get one account per page. Also click on "Print on Bottom of Page" to force the footer to print at the bottom, instead of right after your details. You can also either suppress the page footer or adjust it as needed.
