TVM /TDE Issue

I have a customer with a TDE 200 and TVM Voicemail system. Tey have regular pots line DIL to a custom service menu on th eTVM, from there callers can dial any extension they are after, Last week they started having issues with people calling in and dialling a busy extension number, you get the "please wait" and then there is silence and the call will terminate after about 10 seconds, the caller does not hear the busy greeting. Internal calls work fine for Busy and no answer, calls comming in thru the custom service greeting get the no answer greeting ok but not the busy greeting when the phone is in use.
Any ideas would be appreciated.

Do a busy no answer fwd on each extension to voicemail is the best way
 
All the extensions have a forward busy/no answer to voicemail for internal and external calls, the only issue is with the forward on busy for an incomming external cal via th ecustom service menu,

Any other ideas?

try setting fwd b/na by the phone itself I remember some weirdness like that and the software said it was set but did not work until I manually locked it in!!
