Turn off banner page in printer class from Microsoft support

Hello. I am using the printing class from the Microsoft support web site. It works perfectly, but it prints a banner page before each of the pages that I am sending to print and I would like to turn that off. I have played around with it, but with no luck. Does anyone have any ideas or has been able to change the code to turn it off? If so, would you please share your solution? I would really appreciate the help.
