Trouble adding a 13th phone to a CICS

All,
I have a CICS 4 X 16 Software revision 4.2. I currently have 2 analog lines coming in and 12 stations hooked up via a 66 block. When I try to add sets 13, 14, & 15 the stations never light up. Is there a programming change that I must make, a wiring change, something that I have not thought of???? Is this a problem with the 66 Block?

Any advise that you could provide would be greatly appreciated

I would suspect the wiring or maybe an amphenol cocked in its seat a bit. Make sure the amp is all the way down..
