transtalk cutoffs/disconnects

I installed a new transtalk radio and 2 9040s. The install went fine. Now, the customer is reporting that about once a day they are getting disconnected during their conversation. I did 3 other similar installs at other sites without complaint. Does anyone have any idea what I should look for? Can this be a power issue? I remember that the phone bases were plugged into strips with many other things in them. Might it be related to the position of the radio module?. Previously this site had 9031s that were removed. I put the new radio near where the old one was. Any help is appreciated. Thx.
 
Some say putting 2 9040 radio modules in the same carrier or in the same calling group can make them fail. Never seen either occur, but worth considering.
