Transition to init state timed out -- exiting shutdown

Trying to shutdown our Hewlett Packard Apollo 750 / 15

Keeps getting to this point right here...
Wait for transition to run-level s (allow 20 seconds)
...Before it exits the shutdown.

Furthermore we cant even get it to start up without getting this prompt...

Could Not Fix File System fack -p
Starting A shell for manual D when file system fixed

Any ideas as to whats going on?
 
How did you try a start up if you haven't beeen able to shut it down? I'm guessing by simply powering it off? If so, you probably have corrupt filesystems, hence the second message. You presumably need to run manual fscks on the raw devices of your filesystems; look at the first column in /etc/fstab for likely candidates.

Otherwise some more information about the version of the OS installed, etc. would help; I'm not familiar with that hardware... is that 15 as in Megahertz? Is it that old?
