Transform Data task not executing

Hello,

I have three source connections going to one destination connection for each time it loops.
Also based on loop counter, two of the source connections are disabled and only one is enabled
the problem is it works till the loop counter is 2, after successfully execution of transform data task for loop counter 2, it fails and i am trying to figure out why since yesterday and is really bothering me.
i did all kinda stuff.. i really need any suggestions to debug, please any ideas
