Transferring A Call To ACD - INVALID!

Scenario:-

User has an external caller on the line and decides to supervise transfer to an agent in a particular department.

User presses trans/conf and dials the Path number.
External caller is now on hold.
User is now hearing music on hold as she is queuing on the path.
User now hears ringing [I assume this is the agents phone ringing]
Ringing is heard for a number of seconds.
Then INVALID is displayed on the screen and NU tone is played through handset.

I am guessing that the agent did not answer the call, but why didnt the call re-queue?

Ringing would not be heard after the caller has queued and begun hearing MOH unless the call has interflowed to another destination.

I would start there.
